On April 26th, 1986, at 1:23:40 AM, Alexander Akimov ordered Leonid Toptunov to press the AZ-5 SCRAM button for Unit Four of the Chernobyl Nuclear Power Plant located near the city of Pripyat, Ukraine. This action was quickly caused by Unit Four undergoing a catastrophic steam explosion and ultimately a complete nuclear meltdown caused by a positive void coefficient, positive SCRAM reactivity, poor reactor design and human error.
